I submit this testimony in strong support of S. 2303, the Promoting Physical Activity for Americans Act. This bipartisan legislation, introduced by Senator Roger Wicker (R-MS) and Senator Richard Blumenthal (D-CT), would require the Secretary of Health and Human Services to publish comprehensive, evidence-based physical activity recommendations at least every ten years, with mid-cycle updates addressing emerging evidence and population-specific concerns. Currently, the Department of Health and Human Services publishes the Physical Activity Guidelines for Americans on an irregular schedule without statutory mandate, leaving the timing and content of updates to administrative discretion. Codifying this process ensures that Americans receive consistent, scientifically current guidance regardless of shifts in political leadership. As the American College of Sports Medicine notes in its formal endorsement of S. 2303, the bill would establish a reliable framework for developing, disseminating, and promoting the Physical Activity Guidelines for Americans based on the latest science (ACSM, 2025). The health of our nation depends on such consistency.

Physical Inactivity and the Economic Rationale for S. 2303

Nurses witness the consequences of physical inactivity every day: patients struggling with type 2 diabetes, managing hypertension with multiple medications, and recovering from cardiovascular events that might have been prevented. The epidemiological data is sobering. According to the Centers for Disease Control and Prevention, obesity prevalence among U.S. adults reached 41.9 percent during 2017 through March 2020, more than doubling since the early 1960s, and severe obesity now affects 9.2 percent of the adult population (CDC, 2025). Only 28 percent of adults meet the combined aerobic and muscle-strengthening physical activity guidelines, and among rural residents the figure drops to roughly 16 percent (Abildso et al., 2023). The economic consequences run parallel to the health consequences; the CDC estimates that obesity accounts for nearly 173billioninannualmedicalexpendituresin2019dollars(CDC,2025).A2024reportprojectedthatobesity−relatedhealthcarecostscouldreach173billioninannualmedicalexpendituresin2019dollars(CDC,2025).A2024reportprojectedthatobesity−relatedhealthcarecostscouldreach9.1 trillion over the next decade (Managed Healthcare Executive, 2026). Physical inactivity lies at the core of this crisis. The Physical Activity Guidelines for Americans, 2nd edition, summarizes compelling evidence that regular physical activity reduces risk of all-cause mortality, coronary heart disease, stroke, type 2 diabetes, and several cancers while improving cognitive function and quality of sleep (Piercy et al., 2018). Yet without a statutory mandate, the guidelines risk becoming outdated at precisely the moment when evidence-based recommendations are most critically needed.

Addressing Opponent Concerns About Cost and Implementation

Some may raise concerns about the financial resources required to convene expert panels, conduct systematic evidence reviews, and publish comprehensive reports every ten years. These concerns deserve a direct response. The cost of producing the Physical Activity Guidelines represents a fraction of the economic burden that physical inactivity imposes on the healthcare system. A systematic review by Gelius and colleagues identified multiple policy approaches for promoting physical activity, concluding that population-level interventions grounded in evidence-based guidelines yield measurable improvements in activity levels and health outcomes (Gelius et al., 2020). The return on investment favors action over inaction. Furthermore, S. 2303 explicitly states that no physical fitness standard established under the Act shall be binding on any individual as a matter of federal law; the legislation creates no mandates, imposes no penalties, and respects individual autonomy. It simply ensures that Americans and the professionals who serve them have access to the best available science. Nurses understand that informed patients make better health decisions. This bill equips every American with that information.

References (Selected)

Abildso, C.
